Advance Care Planning (ACP) Workshop

Hospice Peterborough’s Advance Care Planning Workshop is offered by a trained volunteer and is a community education program designed to help individuals reflect on, discuss, and document their future healthcare wishes. The workshop guides participants through important conversations about personal values, treatment preferences, and substitute decision-making, helping ensure their care aligns with what matters most to them if they are unable to speak for themselves. By increasing awareness and understanding of advance care planning, the program supports individuals and families to make informed decisions, reduce uncertainty during health crises, and improve communication with loved ones and healthcare providers.

Palliative Pain and Symptom Management (PPSM)

The PPSM program strengthens the quality and capacity of palliative care across Peterborough, Northumberland, Haliburton, and Kawartha Lakes. Through consultation, mentorship, education, and collaboration, the Palliative Pain and Symptom Management Consultant (PPSMC) supports healthcare providers in long-term care, retirement homes, and community agencies to build confidence and competency in delivering a palliative approach to care for people living with life-limiting illness.

A core component of the program is interdisciplinary education, with training opportunities ranging from foundational hospice palliative care principles (Fundamentals of Hospice Palliative Care), to advanced communication and symptom management skills for personal support workers (APPS), comprehensive palliative nursing education for RNs, RPNs, and NPs (CAPCE), and evidence-based pain assessment and pharmacological management (Essential Pain Management). Together, these programs strengthen workforce capacity and improve the quality of palliative care across care settings.

Clinical Coaching

The Clinical Coach partners with community and healthcare organizations to strengthen palliative care practices through quality improvement initiatives. By supporting organizations to identify priorities, implement practice changes, and evaluate outcomes, the Clinical Coach helps improve the quality and accessibility of palliative care across the health system. The role also connects organizations to education, resources, and funded training opportunities to build sustainable palliative care capacity across the community.

Rural Support

Hospice Peterborough has partnered with Hospice Norwood to provide programming including caregivers support, grief chat, and moving with grief.

Hospice palliative care aims to relieve suffering and improve the quality of living and dying. It strives to help individuals and families to:

  • address physical, psychological, social, spiritual and practical issues, and their associated expectations, needs, hopes and fears
  • prepare for and manage end-of-life choices and the dying process
  • cope with loss and grief
  • treat all active issues and prevent new issues from occurring
  • promote opportunities for meaningful and valuable experiences, and personal and spiritual growth

The term hospice palliative care is often used interchangeably with palliative care.
