What is Palliative Care?

    Palliative care is aimed at the relief of suffering and the improvement of the
    quality of life for persons who are living with or dying from advanced illness or are
    bereaved.  Palliative care aims to treat all active issues, prevent new issues from
    occurring and  promote opportunities for meaningful and valuable experiences,
    personal and spiritual growth and self-actualization. (CHPCA 2002)

What is the Palliative Pain and Symptom Management Consultation?

The primary element of the program is a core team of health care professionals who specialize in palliative care and who offer assessment and management of palliative pain and symptoms. The core team includes physicians, nurses, pharmacists, social workers and pastoral staff and a consultant. The team is available to support the consultant who educates an mentors health care providers throughout Peterborough, Northumberland, City of Kawartha Lakes and Haliburton counties. The overall program is carried out by a collaboration of local hospital and palliative care organizations within these four counties.

Who does the program serve?

The program serves those who are seriously ill by providing specialized palliative care support and consultation to community health care providers such as family physicians, nurses, Access Centre case managers, and staff of long term care facilities.

What is the goal of the service?

The goal of the Palliative Pain and Symptom Management Consultation is to provide a service that is comprehensive and readily available throughout Peterborough, City of Kawartha Lakes, Northumberland and Haliburton counties.

How does the service help?

  • Linking community health care providers to the core team of palliative care consultants who provide advice and recommendations on palliative cases of a complex nature.
  • Educating health care professionals and community groups through lectures, special publications and presentations.
