Hospice Peterborough welcomes you to refer clients and families to our services. The Palliative Care Community Team (PCCT) includes Palliative Nurse Navigators and a Supportive Care Counsellor. These Hospice staff assist with health system navigation, advocacy, counselling and connection with other Hospice Peterborough services. The PCCT can facilitate developing a Circle of Care to involve and support clients and their families and while networking other members of the Health Care Team to ensure consistency of care and connections with other helpful resources. While “every door is the right door,” the PCCT can help navigate the service options available in this region.
