Hospice Peterborough supports health care providers through a number of different programs and initiatives.

Online Referral Process – Health Care Providers are encouraged to download our referral forms and submit referrals by fax (705-742-0064) or phone (705-742-4042).

Health Care Providers are welcome to participate in:

Hospice Palliative Care Community Rounds – Hospice Peterborough hosts monthly rounds for Health Care Providers interested in learning more about hospice palliative care at noon, the fourth Thursday of each month. These Rounds provide a forum for the interdisciplinary team to share ideas, knowledge, initiatives, and decisions impacting on the care of our patients and their families. Contact to be included on our mailing list for rounds email.

Professional Debriefing – Debriefing sessions, facilitated by our staff, are offered upon request to the health care community to help professionals deal with issues that result from the work they do. Hospice offers a safe, confidential space to identify, express and honour the emotional pieces of the stories professionals have carried away from clients and situations. Debriefing sessions, hosted here at Hospice, are intentionally offered offsite from your workplace in order to provide space and safety to this work. Interdisciplinary Education – We offer a number of certificate level courses in different locations across the Northeast Cluster of the Central East LHIN. Please contact the PPSM Administrative Assistant at 705-742-4042 for more information.

The Palliative Pain and Symptom Management Consultation Service (PPSMC) provides specialized palliative care support and consultation to community health care providers such as family physicians, nurses, Access Centre case managers, and staff of long term care facilities.

The service is available throughout the counties of Haliburton, Peterborough Northumberland and the City of Kawartha Lakes. Any healthcare provider may contact the PPSM service by completing and faxing (705-742-0064) the PPSM Referral Form.
